Samp announced the deal this morning, parting with a nominal fee given Conti had less than six months on his Milan contract.
The 27-year-old right-back was signed from Atalanta for €24m in 2017, but his career at San Siro was destroyed by a series of injury problems.
Conti made just one appearance for Milan this season, for 12 minutes in the 3-1 Serie A victory over Roma on January 6.
Overall, Conti managed only 52 competitive games for the Diavolo, in which he contributed five assists.
