The outlet states that the 17-year-old has agreed terms on a three-year contract with the Stamford Bridge club.
Samuels-Smith is an impressive youngster, having excelled in the youth setup at Everton.
He is now set to move for a fee in the region of 4 million, which could include add-ons as well.
Chelsea have also secured Alex Matos from Norwich City, while Diego Moreira has signed from Benfica.
