
- Four faults have been found in the safety system of a train involved in a fatal crash near Talerddig, Powys, Wales, on October 21, 2024.
- The crash involved two Transport for Wales trains, resulting in one fatality and 23 minor injuries. The driver and guard of the trains were seriously injured.
- Sanding hoses, designed to improve braking, were blocked with leaves and debris. Two flow rate plates were also incorrectly installed. Two electrical faults were also discovered.
- The train with the faulty system slid for 900 metres before colliding head-on with another train.
- An investigation is underway, focusing on the sanding system and the low adhesion conditions at the crash site.
