The topic about sugar-free drinks have always been controversial in the society today. Sugar-free drinks, often marketed as healthier alternatives to sugary beverages may not seem as healthy as they do just because they did not add sugar, their use of artificial sweeteners like aspartame, sucralose, and saccharin isn't a healthy option anyway. While these sugar substitutes are low or zero-calorie and can help reduce overall sugar intake, growing research raises concerns about their long-term health effects. Studies from the Institute of Science and Technology suggest that artificial sweeteners may disrupt gut microbiota, increase insulin resistance, and even paradoxically lead to weight gain and type 2 diabetes. There are also debates around potential links to cancer and neurological effects, though findings remain inconclusive.
According to See Hua News, a 67 year old woman surnamed Chen from Hubei province China has became inactive after retiring. She would spend her mornings scrolling on her phone and watching Douyin (the Chinese version of TikTok) on the sofa, and play online mahjong from noon to late at night, living an extreme sedentary lifestyle. She rarely leaves her seat except for meals and bathroom breaks.
Concerned for her health, her son tried to help her out but made things worst. He bought her multiple sugar-free cola and “zero calories and zero sugar” electrolyte drinks, hoping that these would be healthier options for his mum instead of relying on normal sugar-added beverages. Unfortunately, due to these marketing terms, Chen got into it and thereafter began replacing plain water entirely with these drinks, consuming more than three bottles daily, thinking that it wouldn't harm her health.
Initially, she experienced blurred vision and weight loss. Later, she developed symptoms such as diarrhea, loss of appetite, and general fatigue. Hospital tests revealed abnormal blood sugar levels. Despite a week of treatment, her blood sugar remained high, and she began to show signs of frequent urination, urgency, and heart palpitations. Her family urgently transferred her to another hospital for treatment. After examination, the attending physician diagnosed her with type 2 diabetes, along with complications such as a urinary tract infection and hypokalemia (low potassium levels).
The doctor explained that Ms. Chen’s age, being overweight, a sedentary lifestyle, and her habit of staying up late were all contributing factors. She already had issues like reduced pancreatic function and insulin resistance, which had gone unnoticed due to a lack of symptoms. Over the past two months, her excessive intake of artificially sweetened drinks may have disrupted her gut microbiome, which is directly linked to insulin resistance and elevated blood sugar levels. Artificial sweeteners can be several times sweeter than sugar, which may trick the brain into thinking sugar is being consumed, prompting insulin secretion. However, since there's no glucose to break down, this "false alarm" leads to reduced insulin sensitivity over time, which then leads to type 2 diabetes.
Similar findings were shared by de Koning et al. in 2011 in his journal article on ScienceDirect, comparing the development of diabetes mellitus type-2 with the consumption of artificially sweetened beverages and sugar-sweetened beverages (SSB). Increased intake of artificial sweetened beverages increased the incidence of type-2 diabetes, suggesting possible mechanism of insulin resistance over time. Similar conclusions were posted on the National Library of Medicine where authors noticed patients who consumed artificial sweetening agents had higher insulin resistance as compared to other patients who had no artificial sweeteners
Luckily, by listening to the doctor's advice of drinking more plain water and adopting a low-salt diet, her condition improved. Regardless of your age, it's important to take care of your diet and avoid depending on just one type of food or drink. A balanced intake is essential for good health.
