In an alarming revelation that raises questions about the transparency and fairness of private healthcare costs, Mark O'Dell, CEO of the Life Insurance Association of Malaysia (LIAM), found himself in disbelief when he received a staggering 13-page bill for a minor hernia surgery. The procedure, performed at a private hospital in Kuala Lumpur, came with a hefty price tag of RM18,837.55, a figure that has left many questioning the ethics of hospital billing practices in Malaysia.

O'Dell, a veteran with over 44 years of experience in the life insurance industry, was left bewildered by the extensive list of charges. Despite his deep understanding of insurance and medical billing, he admitted that many of the items listed were incomprehensible to him. He lamented that a straightforward procedure with just a one-night hospital stay resulting in a 13-page bill is perplexing, a concern shared by many Malaysians who have encountered similar situations.

The bill, issued in late May 2024, was not just lengthy but also filled with charges that seemed unnecessary or inflated. "You look through all the items, and I can’t make sense of some of them," O'Dell remarked during a recent interview with 'CodeBlue'. His frustration echoes a broader sentiment among patients and insurers alike, who often find themselves at the mercy of private hospitals' opaque billing practices.

O'Dell's experience has led him to advocate for a more regulated approach to private hospital billing, specifically through the implementation of Diagnostic Related Groups (DRG).

What is the DRG System?

The DRG system is a healthcare payment model where hospitals are paid a fixed amount for treating patients based on their diagnosis and procedure, rather than itemizing every charge. This method promotes cost efficiency and reduces unnecessary charges by giving hospitals a set budget for each case.

Regulating Private Hospital Bills Through DRG

Contrary to the views of Dr Kuljit Singh, President of the Association of Private Hospitals Malaysia (APHM), who claims that private hospital bills cannot be regulated, O'Dell firmly believes otherwise. He argues that DRGs could be effectively implemented in Malaysia’s private healthcare sector, just as they have been in public systems worldwide. "Private hospitals do not want this. They will make more profit by having these line item bills," he explained, highlighting the profit-driven motives that often underlie complex billing systems.

O'Dell’s call for regulation draws attention to the urgent need for the Ministry of Health (MOH) to step in and mandate the use of DRGs across private hospitals. He suggests that these regulations could mirror those already in place for doctors' fees under the Private Healthcare Facilities and Services Act (PHFSA) 1998, which caps consultation fees for private clinic GPs and specialists.

Citing past public-private collaborations during the Covid-19 pandemic, where DRG-like systems were successfully used to manage hospital resources, O'Dell remains optimistic that change is possible. He affirmed that with the will to make it happen, it can be achieved.

Mark O'Dell’s experience serves as a stark reminder of the pressing need for greater transparency and fairness in Malaysia's private healthcare system. As more patients and industry leaders call for reform, the question remains: will the government take the necessary steps to protect consumers from exorbitant medical bills?
