NHS staff ‘lost’ schizophrenic patient just months before he killed three people

  • Schizophrenic Valdo Calocane was discharged from mental health services just months before he killed three people in Nottingham, an inquiry has heard.
  • He had been under the Nottinghamshire Healthcare NHS Foundation Trust's supervision for two years, with a consultant psychiatrist previously warning he would kill someone.
  • The trust's Early Intervention in Psychosis (EIP) service discharged Calocane in September 2022 after he failed to attend appointments and could not be found, the inquiry heard.
  • He also lied about being abroad in his last phone engagement, the inquiry heard, with Emma Robinson, a team leader at the trust, admitting that staff “lost” him.
  • The trust did not inform Nottinghamshire Police of Calocane's discharge, and his GP reportedly received very little information about his mental health history.
