When most people think of plague, they think of the Black Death. Rats. Fleas. Crowded medieval cities. Mass graves across Europe. But a study published in Nature in June 2026 found that plague was already a deadly killer thousands of years before any of that existed.

An international team of researchers analysed ancient DNA preserved in the teeth of 46 individuals buried at four prehistoric cemeteries along the Angara River in Siberia, near Lake Baikal. The people who lived there were hunter-gatherers, small mobile communities with no agriculture, no dense settlements, and no contact with the urban environments that later pandemics would exploit.

Out of 46 individuals, 18 carried DNA from Yersinia pestis, the bacterium that causes plague. That is 39%. The researchers identified two previously unknown strains of the bacterium, both dating back roughly 5,500 years. The outbreaks occurred in two separate waves, one between 5,596 and 5,341 years ago, and a second between 5,156 and 4,706 years ago.

The ancient strains lacked the gene that enables flea-borne transmission, the mechanism that made later plague pandemics so devastating. But they were clearly lethal. Many of the infected individuals were children and young teenagers. Some were buried in mass graves with evidence of rapid, family-based outbreaks. These were not isolated infections. They were concentrated kill events.

"Whether the earliest forms of plague were mild or virulent has been a matter of debate, but our findings demonstrate that these ancient strains were already highly lethal," the researchers noted in their paper.

Previous studies had identified isolated ancient individuals infected with Yersinia pestis, including a case from Sweden dated to 4,900 years ago and another from Latvia at around 5,000 years ago. But this is the first evidence of plague at outbreak scale, affecting entire communities rather than single individuals.

The discovery rewrites the timeline of one of humanity's most feared diseases. Plague did not wait for cities. It did not need rats, fleas, or grain stores. It was already finding ways to spread through small, scattered human groups living thousands of kilometres from any urban centre. The disease was lethal long before it became efficient.

The research was led by genomicist Ruairidh Macleod of the University of Oxford and involved an international team including researchers from the University of Copenhagen, the University of California Santa Cruz, and institutions in Russia. The ancient DNA was extracted and sequenced from teeth, which preserve bacterial DNA more reliably than bone over thousands of years.

If a pathogen that devastating could move through tiny hunter-gatherer communities 5,500 years ago, what does that say about our assumptions of how diseases spread? We tend to associate pandemics with density, with cities, with global trade routes. But plague did not need any of that. It found a way through small, scattered groups living on the shores of a Siberian lake, long before the first city wall was ever built. That is a question worth sitting with.
