- The watchdog is examining delays in Levy's handling by police after he was bailed despite being arrested for sexual assaults on the London Underground in late 2024.
- A separate IOPC probe remains ongoing into two Metropolitan Police officers regarding their management of Levy after his release on licence in February 2023.
- The Metropolitan Police, British Transport Police and Crown Prosecution Service have all admitted failings in how they dealt with him.
