
The Malaysian Ministry of Health (MOH) is considering adopting a diagnosis-related groups (DRG) payment system to address the escalating costs of private healthcare. Health Minister Datuk Seri Dr. Dzulkefly Ahmad views the DRG, or its variant case-related groups (CRG), as a potential solution to standardize private hospital billing and curb medical inflation.
“This system represents the way forward. Whether through DRG or CRG, the focus would be on value-based care,” Dzulkefly stated at the launch of the Association of Private Hospitals Malaysia (APHM) Factbook 2024. By implementing such a system, the government aims to establish transparent and predictable billing practices while addressing the growing financial burden on patients.
Tackling Rising Costs
Private healthcare costs have become a pressing concern for Malaysians. Mark O’Dell, CEO of the Life Insurance Association of Malaysia (LIAM), emphasized that DRG implementation could mirror regulations governing doctors’ fees under the Private Healthcare Facilities and Services Act 1998. DRG rates could be tailored based on regional and institutional factors, as seen during the public-private collaborations amid the Covid-19 pandemic.
Challenges in Perception
Despite the benefits, APHM President Dr. Kuljit Singh stressed that misconceptions about private hospitals’ profit motives often overshadow their contributions to the healthcare ecosystem. He argued that private hospitals maintain price transparency and adhere to strict safety and quality regulations. Furthermore, they are significant economic contributors, generating RM12 billion annually for Malaysia’s GDP, including RM2 billion in insurance payments and substantial revenue from medical tourism.
Striking a Balance for Affordable and Ethical Healthcare
The DRG system holds significant promise, but its success hinges on effective execution and a fair balance between patient affordability and the sustainability of private hospitals. Ensuring transparent pricing while maintaining high standards of care is vital to countering perceptions of profiteering in the private healthcare sector.
Standardized packages can make a meaningful difference for patients by providing clear and predictable costs, ensuring quality care remains accessible at a “reasonable” price. For healthcare providers, such measures encourage a shift toward ethical practices, prompting doctors to minimize unnecessary procedures and focus on delivering real value to patients.
By fostering a more transparent and value-driven system, the DRG approach has the potential to not only address financial burdens but also reinforce healthcare as a compassionate service rather than a business.
As MOH mulls over this system, stakeholders await a definitive decision. For many Malaysians, the hope lies in a healthcare framework that ensures fair pricing without compromising quality, making private care accessible to all.
