  • Senior Coroner Alison Mutch concluded Madison died while asleep in a prone and unsafe position, highlighting the risks posed by unregulated services and untrained individuals.
  • The coroner issued a prevention of future deaths report to the Secretary of State for Health, urging regulation of maternity services to prevent similar tragedies.
