Sleep. That simple, universal necessity. It’s the cornerstone of health, yet how we perceive it and how we actually experience it often seem at odds, especially between men and women.

In recent years, disturbed sleep has emerged as more than just a nuisance. It’s been firmly linked to early mortality and a raft of serious conditions like diabetes, cardiovascular disease, even neurological decline.

The stakes are high, but the story of sleep is more nuanced than most realise.

Women frequently report poor sleep. They often fill waiting rooms in sleep clinics, raising concerns about restless nights and weary mornings. Oddly enough, objective measures suggest a different narrative.

Some studies indicate that men, not women, may actually have worse sleep quality when measured with scientific instruments. It’s a paradox that has confounded sleep researchers and clinicians alike.

A study published previously in Sleep Advances and Journal of Sleep Research has peeled back the layers of this puzzle. Scientists in the new study also set out to directly compare subjective ratings and objective measures of sleep between men and women, aiming for clarity where confusion reigns.

The experiment was robust: 238 women and an equal number of men, matched carefully by age and BMI. These participants weren’t plucked from clinics but randomly selected, ensuring a fair cross-section of society.

Each participant slept in their own home, free from the artificial constraints of a laboratory. Their sleep was meticulously recorded using electroencephalography (EEG), electromyography (EMG), and electrooculography (EOG)—tools that trace brain waves, muscle tension, and eye movements. This allowed scientists to monitor sleep stages, time spent awake, and how quickly people drifted off.

The morning after, participants rated their own sleep. How difficult was it to fall asleep? Was their sleep restless? Did they wake up early? How many times did they believe they woke up? How long did it take to fall asleep? How long did they sleep in total? And most importantly, how would they rate their overall sleep quality?

Sleep technicians then scored the recordings, visually inspecting the data to establish the objective reality. All results were adjusted for key factors such as age, gender, alcohol consumption, smoking, ensuring no confounding variables skewed the findings.

The results were revealing. Women consistently rated their sleep quality lower than men did.

Yet the data told a different story. Objectively, women enjoyed fewer nighttime awakenings, less superficial stage one sleep, higher sleep efficiency (more time asleep while in bed), deeper stage three sleep, and longer total duration (400 minutes compared to men’s 382 minutes). In nearly every measurable aspect, women appeared to have better quality sleep.

There was only one catch. When women did wake up during the night, they tended to spend more time awake—about nine minutes each time, compared to men’s seven. This detail is significant. Research shows that if someone spends at least five minutes awake during the night, they’re likely to remember it the next day. Thus, women may be more accurately recalling their nocturnal awakenings and estimating their frequency.

Men, on the other hand, grossly underestimated how often they woke up—by 72%, compared with women’s 37%. This discrepancy is striking. Scientists believe that men’s shorter awakenings mean they’re less likely to remember these interruptions, leading them to overestimate their sleep quality.

When it came to concrete metrics like time to fall asleep, total sleep duration, and time spent awake, both sexes were fairly accurate in their self-assessments. However, the real difference lies in the subjective experience of waking up at night.

Researchers delved deeper. They found that men who woke up only briefly during the night, about eight minutes or less, often didn’t recall these interruptions at all. When this group was excluded from analysis, the gender gap in subjective sleep quality vanished. This suggests that men with short nighttime awakenings report better sleep simply because they don’t remember having been awake.

Age added another layer to the story. Men’s objective sleep deteriorated faster with age than women’s did, especially in stage three (deep) sleep. Women over 65 received around 80 minutes of deep sleep nightly; men, only 53 minutes. Among younger adults (30 to 50 years old), the difference was negligible (about 70 minutes for both).

This research underscores an important truth. Sleep quality isn’t just about physiological measurements. It’s also shaped by our perceptions and memories. The experience of awakenings wields significant influence over how we judge our own sleep.

Why does this matter? For women, longer periods awake during nocturnal interruptions make these events harder to forget, leading them to believe their sleep was restless or insufficient, even if objective measures say otherwise. For men, shorter interruptions slip beneath the radar of memory. Their self-assessment paints a rosier picture than reality would suggest.

But there are consequences. If men routinely overestimate their sleep quality because they don’t remember waking up, they may overlook symptoms or conditions that need medical attention. Sleep disorders can be stealthy; missing early signs may allow problems to worsen unchecked.

Similarly, women may report worse sleep not because their bodies are suffering but because their minds remember every fragment of wakefulness. This doesn’t mean their complaints are invalid—perception is powerful and influences well-being but it does highlight the complexity of diagnosing and treating sleep-related issues.

The findings prompt new questions for science and medicine. If common sources of disturbed sleep like alcohol use, smoking and BMI were adjusted for in this study, what else could be driving poor sleep quality among men as they age? Why does deep sleep diminish more rapidly in older men than in older women? And crucially, would these patterns hold true if participants were observed for more than one night?

Future research should aim for longer-term studies, capturing fluctuations in sleep across weeks or months. Experts suggest that exploring psychological factors such as stress levels, anxiety, cultural expectations around masculinity and vulnerability which may also contribute to how men perceive their own health and rest.

Sleep clinics may need to rethink how they assess patients. Objective screening tools are essential; relying solely on self-reported symptoms could mean missing hidden problems in male patients or overestimating difficulties in female patients.

For public health professionals and policy-makers, these insights carry weight. Raising awareness about the gap between perceived and actual sleep could help people seek appropriate support sooner. It could also inform targeted interventions for older adults, especially men to preserve deep restorative sleep.

The story of sleep remains unfinished. As science advances, so too will our understanding of its mysteries—and perhaps our ability to bridge the divide between perception and reality.
